COMMUNITY COMMITMENT: New Transitional Home for the Homeless

RRH is excited to announce we have relocated our Transitional Housing program, a program that extends care for homeless individuals beyond our hospital walls and into the community.  In partnership with DePaul, RRH has opened a new transitional home for the homeless in the Corn Hill district off Troup Street.

The new Corn Hill transitional home features eight units that accommodate patients discharged from Rochester General, St. Mary’s or Unity, while the team works to attain permanent housing. The individuals receive supportive services including:

  • Health Home Care Management
  • Homecare
  • Meals
  • Medication reminders
  • 24/7 supervision

Over the past three years, the RRH and DePaul partnership has successfully placed 100+ patients in permanent housing.
