WELL DONE: Unity Hospital Receives $3.1 Million Grant from New York State Department of Health


Last week, Governor Andrew Cuomo released a list of recipients of state grants to help create financially stable health care systems. The grants are part of the Statewide Health Care Facility Transformation Program.

 

We are proud to announce that Unity Hospital will receive $3.1 million for the expansion and consolidation of Unity Family Medicine, the Health Reach Program (Healthcare for the Homeless), a Unity OB/GYN practice, West Main Pediatrics, and a patient service center for collection of lab testing specimens. The funds will also support the transformation of our delivery model to facilitate integrated health care and team-based medicine for our most vulnerable populations.

 

This project will increase primary care capacity to better meet patient demand, facilitate an integrated network of providers that is focused on value and health outcomes, integrate behavioral health with primary health, and support the communication and collaboration among key partners needed to expand care management and practice transformation.

 

Unity Hospital applied for the grant in September 2016. Written confirmation of the award is expected from the New York State Department of Health in the near future. Conversations are underway to determine a timeline for this project.
