Proven model for success: For over two decades, ElderONE has been providing seniors customized care plans and coordinated services, allowing them to stay in their own homes without compromising their health or safety. Now for the first time, ElderONE is expanding its innovative model of care to residents in Wayne and Ontario Counties. A grand opening for the new Silver Hill PACE (Program of All-Inclusive Care for the Elderly) Center in Newark will be held on Wednesday, June 7 at 10 a.m.
This is Rochester Regional Health’s fourth PACE center. The expansion reinforces our mission and commitment to bringing exceptional care to neighborhoods of the families we serve.
ElderONE, an Affiliate of Rochester Regional Health, is a nationally recognized model of managed long-term care called PACE (Program of All-inclusive Care of the Elderly) that has served the region since 1990. The program cares for seniors ages 55 and older who have physical and medical needs that might otherwise cause nursing home placement. A full team of healthcare professionals provides a comprehensive range of services paid by Medicare and Medicaid. The PACE Center is the team’s central hub, with home care, transportation, prescription medications, and other services supporting the senior in their own home.
